Why can malignant tumors be operated, and the operation is the first choice? The medical profession believes that this is just like removing the mountain (the main body of the tumor) first, and then cleaning up the surrounding small clods with chemotherapy and radiotherapy. It is an effective treatment mode, which is called "tumor-removing surgery" academically.

Now, some experts at home and abroad think that this treatment mode is outdated, at least for patients with advanced cancer, it cannot be treated according to this mode. Zhu Zhenggang, an expert in the diagnosis, treatment and research of gastric cancer in the gastrointestinal surgery department of Shanghai Ruijin Hospital, suggested that patients with advanced gastric cancer should not be operated on, because patients with advanced gastric cancer will relapse soon after operation and their survival time is very short. On the contrary, we should take another method, chemotherapy first, then surgery.

This new concept and practice of treatment came out a few years ago abroad and is called "transformation therapy", which is based on clinical research. The new cancer treatment method subverts the traditional concept of cancer treatment, that is, first "transform" the tumor, turn the large tumor into a small tumor, and turn the late tumor into a middle stage or even an early stage, and then perform surgical resection.

The tumor transformation method before operation is chemotherapy. This new treatment method tried by Shanghai Ruijin Hospital is called "preoperative neoadjuvant intraperitoneal combined with systemic chemotherapy" (NIPS), which has been used for more than a year. The treatment results are surprising: the tumor cells that had spread in a large area disappeared, the ascites disappeared, and the fist-sized tumor was reduced to the size of a bean … A group of advanced patients underwent tumor resection after several courses of NIPS treatment, and the survival period was more than one year. Previously, doctors predicted that their survival time was only 3-6 months.

Based on this result, as a surgeon, Zhu Zhenggang went around to "stop the knife". He thought it was useless to rush to operate, and it would be counterproductive. Some doctors in Peking Union Medical College Hospital, Shanghai Zhongshan Hospital, Shanghai Cancer Hospital, Beijing Cancer Hospital and other big hospitals are also changing their concepts and not easily operating on patients with advanced gastric cancer.

Why do doctors such as Zhu Zhenggang believe that chemotherapy before surgery can better treat patients with advanced cancer? This is scientifically based, and it is the result of evidence-based medical treatment conducted by foreign doctors in clinic.

Second, the concept of evidence-based medicine holds that the credibility of expert opinions is the lowest.

Many diseases find the best treatment in the process of trial and error, and the above new treatment for gastric cancer was put forward only after obtaining the evidence, which involves a new concept of modern medicine-evidence-based medicine (medical treatment).

Evidence-based medicine means "evidence-based medicine", also known as empirical medicine, which is called evidence medicine in Hong Kong and Taiwan. It was put forward in 1996 by sakit, a clinical epidemiologist at the Evidence-based Medicine Center of the National Health Service Department, John Radcliffe Hospital, Oxford University, UK. The representative monograph is How to Practice and Teach Evidence-based Medicine. In the latest edition (2000 edition) of this monograph, sakit defined evidence-based medicine as "prudently, accurately and wisely applying the best research basis available at present, combining the personal professional skills of doctors and years of clinical experience, and considering the values and wishes of patients, and combining the three perfectly to formulate the treatment measures for patients".

The basic content of evidence-based medicine can be summarized as a core concept, two gold standard evidences, three basic elements, four basic steps and five-level evidence gradient.

A core concept is that doctors should stress and rely on evidence in treating diseases, and the evidence comes from the latest high-quality clinical research results, which is external evidence relative to personal experience.

However, the evidence is true or false, so it is necessary to have reliable or important evidence, that is, two gold standard evidence. One is the systematic review (RCT) of multiple randomized controlled trials, that is, systematic review or meta-analysis; The second is a randomized controlled trial with a large enough single sample (case), which are the most reliable evidence to prove the effectiveness and safety of a certain diagnosis and treatment method.

Three basic elements mean that clinicians should rely on three basic elements to formulate the best diagnosis and treatment plan for different patients, namely, the latest and best clinical research evidence collected, the doctor’s skilled clinical experience and the individual situation and wishes of patients.

The four basic steps are to ask questions in clinical practice, search for evidence all over the world, evaluate the latest and most comprehensive evidence collected, then apply the best evidence, and then evaluate the curative effect.

The five-level evidence gradient means that the research evidence is divided into five levels according to quality and reliability, and the strength of the evidence decreases in turn. The first level is a systematic review of all RCTs, the second level is RCTs with a large enough sample size, the third level is a non-randomized controlled study, the fourth level is an uncontrolled series of case observations, and the fifth level is an expert opinion.

Sakit believes that in clinical treatment, we should first consider the use of one or two levels of gold standard evidence, and in the absence of gold standard evidence, other levels of evidence can be used in turn, and expert opinions have the lowest credibility.

According to the above-mentioned concept of evidence-based medicine, there should be evidence for the treatment of advanced gastric cancer or other advanced cancers. According to the evidence of Ruijin Hospital, more than 30 cases of advanced gastric cancer have been treated by NIPS, and the success rate of surgical transformation has reached 70%, of which 77.8% have been completely removed. After conversion treatment, the survival time of patients has reached more than one year, and the quality of life is also good.

However, according to the second of the two gold standards of evidence-based medicine, that is, a randomized controlled trial with a large enough single sample (case), there are only more than 30 cases of evidence in Ruijin Hospital. Therefore, more cases are needed to prove the effectiveness of this treatment.

Third, the era of specialists working in isolation is out of date.

In the diagnosis and treatment of cancer, doctors should also follow the treatment guidelines formulated by medical professional associations, authoritative national medical research centers or authoritative national health administrative departments, which are also based on the results of evidence-based medical research.

NCCN is the abbreviation of American National Comprehensive Cancer Network. It is a non-profit alliance organization composed of 25 famous cancer centers in the world (including NCI in the United States). It publishes various clinical practice guidelines for malignant tumors every year. Because of its universality and authority, it has been widely recognized and followed by clinicians all over the world.

The Ministry of Health of China has also issued relevant guidelines for cancer diagnosis and treatment, such as the Guidelines for Diagnosis and Treatment of Gastric Cancer (2011 Edition).

However, in the clinical diagnosis and treatment, there are still many hospitals and doctors in our country who do not treat diseases completely according to the guidelines, but fight for their own treatment with their own clinical experience and specialty characteristics. Similarly, taking gastric cancer as an example, if patients with gastric cancer seek treatment in different departments of the hospital, such as surgery, oncology, radiotherapy, etc., these departments will treat the patients with surgery, chemotherapy and radiotherapy respectively, which does not meet the requirements of the guidelines for the diagnosis and treatment of gastric cancer and lacks the overall treatment concept.

In order to change this situation, the Sixth Affiliated Hospital of Sun Yat-sen University made a new attempt. They set up a multi-disciplinary clinic for gastric cancer, where specialists such as gastrointestinal surgery, oncology, radiotherapy, thoracic surgery, interventional department and pathology gathered together to discuss each case of gastric cancer and reach an agreement on the final diagnosis and treatment plan of the patients. This combined outpatient service can provide patients with personalized and optimized treatment plans, and is also suitable for the treatment of other cancers and diseases.

Xu Ting’s Death and Medical Choice

On the afternoon of September 7th this year, Xu Ting, a 26-year-old post-90s actress, died of malignant lymphoma in Beijing 304 Hospital, after several months of treatment.

Xu Ting’s death shows that in the face of cancer, human ability is still limited. However, if we actively treat cancer and choose appropriate treatment methods, we can still greatly reduce the mortality rate after cancer, and achieve the "three one-thirds" goal of cancer prevention and treatment put forward by the World Health Organization, that is, one-third of cancer can be prevented; One third of cancers can be found and cured early; One third of cancer patients can relieve their pain, prolong their lives and improve their quality of life through effective comprehensive treatment.

Xu Ting’s death shows that she didn’t even reach the latter 1/3 goal of the World Health Organization. Why? A large number of media reports have outlined the initial reason, which is the result of Xu Ting’s personal choice.

In May this year, a hospital in Nanjing initially diagnosed Xu Ting with cancer, but the whole family, including Xu Ting, didn’t believe it.

After that, on July 9th, Xu Ting exposed the examination reports from Beijing 301 Hospital and Beijing Cancer Hospital in Weibo, and all the examination reports from the two hospitals pointed to malignant lymphomas.

Although both hospitals advised her to undergo chemotherapy, Xu Ting refused after consideration. This is a choice based on Xu Ting’s personal life experience. This choice has also prompted Xu Ting to go to two forks in the road. One is that he does not intend to find out what kind of cancer he is suffering from through western medicine, and the other is that he is unwilling to undergo chemotherapy. Because she had seen Qin Sihan, the "most handsome traffic policeman in Chengdu", who suffered from leukemia chemotherapy, her hair fell out and she was weak, but she finally passed away. She believes that adopting western medicine treatment will lead to both human and financial resources.

In fact, Xu Ting suffers from T-lymphoblastic lymphoma, which is a very deadly malignant tumor. Although the treatment of this malignant lymphoma is controversial at home and abroad, it is agreed that combined chemotherapy and individualized treatment of autologous hematopoietic stem cell transplantation can make some patients get better therapeutic effect and longer survival time. However, Xu Ting chose Chinese medicine treatment based on his personal life experience, including cupping, scraping, acupuncture and bloodletting.

Chinese and Western medicine have their own characteristics and indications. Cupping and scraping are helpful to dispel fatigue. In this year’s Olympic Games, online celebrity has been transformed by American swimmer Phelps, but it is obviously not suitable for the treatment of T-lymphoblastic lymphoma, and it has no symptomatic effect. Acupuncture helps to relieve pain, but bloodletting has not been proved to be effective for any specific disease so far. Therefore, these methods are totally different for the treatment of Xu Ting’s disease, and even seek fish from the edge of the wood.

Although Xu Ting didn’t always be stubborn, he finally returned from the fork in the road and received chemotherapy. The results of chemotherapy completely refuted her personal experience. She didn’t lose her hair like Qin Sihan and her weight didn’t change. And has a preliminary curative effect.

However, it’s all too late. Xu Ting received chemotherapy only one week before his death. According to the experience of clinicians, T-lymphoblastic lymphoma will spread far away in the early stage, invading bone marrow and central nervous system. Therefore, even early patients should be treated as stage IV (advanced stage) for chemotherapy. Before she died, Xu Ting’s mother asked her if she regretted chemotherapy now. She said, regret it! This is enough to explain the problem.

Xu Ting’s death certainly illustrates the danger of cancer, but it is also extremely important to adopt what kind of treatment plan after suffering from cancer. The question is, who will spread the correct knowledge of cancer prevention and treatment to Xu Ting and the thousands of Xu Ting in Qian Qian? This is the most important thing.

Lessons from the surge of cancer patients after September 11th.

  Administrative measures for examination and approval

  In order to further standardize the management of construction funds for major provincial-level education, culture and health projects, and clarify the examination and approval procedures and authority, according to the Notice of the General Office of the Provincial People’s Government on Printing and Distributing the Decision-making Procedures for Major Projects at Hunan Province Level (Trial) (No.23 [2015] of Hunan Zhengban) and the Notice on Printing and Distributing the Implementation Rules for the Decision-making Procedures for Major Projects at Hunan Province Level (No.58 [2015] of Hunan Zhengban)

  I. Scope of examination and approval

  Major provincial-level education, science, culture and health projects (hereinafter referred to as major projects) refer to public welfare facilities construction projects that need provincial-level financial funding arrangements (including debt funding arrangements with provincial-level financial funds as the main source of debt repayment, the same below) in the provincial-level education, science and technology, culture, sports, medical and health care, family planning and other industries and are included in the decision-making scope of the provincial people’s government. Specifically including:

  (a) the provincial total investment of 50 million yuan and above, culture, sports, science and technology and other large venues construction, renovation and expansion or asset purchase projects;

  (two) the provincial universities, public hospitals and other new construction, renovation and expansion, overall relocation or asset purchase projects with a total investment of 100 million yuan or more at the provincial level;

  (three) other major projects required by the provincial people’s government to be included in the scope of examination and approval.

  Second, the basic principles of examination and approval

  (1) Strict procedures and standardized examination and approval. Major projects shall specify the investment scale and sources of funds in advance. Audit of funding arrangements should be regarded as a necessary procedure for planning and project decision-making. Without audit of funding arrangements, it shall not be submitted to the provincial people’s government for deliberation and decision-making.

  (two) intensive economy, to avoid duplication. Major projects should consider the needs of career development and the possibility of financial resources as a whole, reasonably determine the scale of investment, and strive to avoid greed for foreign countries. Strictly control the demolition of old buildings and the construction of new ones. The new construction, reconstruction and expansion of projects should be connected with existing related facilities, and those with similar functions should be integrated as a whole, intensively utilized and saved funds. Encourage qualified projects to adopt the mode of provincial and municipal co-construction.

  (3) Coordinating the stock and ensuring the key points. The funds needed for the construction of major projects are first raised from the existing special funds at the central and provincial levels and the stock funds of project construction units. Provincial departments in charge of industry should co-ordinate the integration of stock funds, give priority to ensuring the demand for funds for major projects, and if the stock funds are insufficient, apply for new arrangements for provincial budgets according to procedures.

  (4) Strict supervision, openness and transparency. Major project construction funds shall strictly control the investment scale in accordance with the principle of estimating and controlling budgetary estimate, budgetary estimate controlling budget and budgetary control final accounts, and shall not overspend in principle. If it is really necessary to increase financial capital investment, it shall be submitted for approval according to the original procedures. The project construction and fund approval shall be subject to social supervision in accordance with the principle of "non-confidential and full disclosure".

  III. Approval Procedures and Authority

  Major project planning, decision-making, budget estimates and other links involving financial arrangements, as well as related debt borrowing and asset disposal and other matters, according to the following procedures for examination and approval:

  (a) approval of funds for special construction planning.

  The special construction planning of education, science, culture and health shall specify the construction standards, construction scale and sources of funds of related major projects during the planning period. Special construction planning is instructive and binding. Except as required by the central authorities and approved by the provincial people’s government, financial funds shall not be arranged for major projects not included in the planning.

  Provincial departments in charge of industry shall solicit opinions from the Provincial Department of Finance when preparing the draft special construction plan. The Provincial Development and Reform Commission shall consult with the Provincial Department of Finance in the preliminary examination of the draft special construction plan, if it involves the arrangement of provincial financial funds. The Provincial Department of Finance, in combination with factors such as the financing planning of major infrastructure construction at the provincial level and the comprehensive balance of revenue and expenditure, reviews the overall investment scale, sources of funds and annual funding arrangements for special construction planning, and puts forward opinions and then feeds them back to the Provincial Development and Reform Commission. Provincial Development and Reform Commission, according to the opinions of the Provincial Department of Finance and other relevant departments, put forward preliminary opinions on special construction planning, and submit them to the provincial people’s government for examination and approval according to procedures; If the preliminary examination opinions are inconsistent with those of the Provincial Department of Finance, special explanations should be made.

  (two) the approval of major project decision-making funds.

  Provincial departments in charge of industry shall solicit opinions from the Provincial Department of Finance when carrying out the preliminary work of major projects. The Provincial Development and Reform Commission shall consult with the Provincial Department of Finance (except for major projects that are fully arranged by the central and provincial budgets for infrastructure investment) when conducting preliminary examination of applications for major projects. The Provincial Department of Finance, in combination with the current situation of existing assets and debts, conducts a preliminary examination of the project construction scale, construction standards, investment estimation, funding sources, annual funding arrangements and other relevant contents related to financial funding arrangements, and puts forward opinions and feedbacks to the Provincial Development and Reform Commission or after approval according to procedures according to the authority stipulated in the Notice of the General Office of the Provincial People’s Government on Printing and Distributing the Administrative Measures for the Examination and Approval of Provincial Financial Special Funds in Hunan Province (No.90 [2015]). Provincial Development and Reform Commission, according to the opinions of the Provincial Department of Finance and other relevant departments, put forward preliminary opinions on project decision-making, and report them to the provincial people’s government for approval according to procedures; If the preliminary examination opinions are inconsistent with those of the Provincial Department of Finance, special explanations should be made.

  (three) approval and adjustment of budget funds.

  1. The budget estimate is approved. After a major project is approved, the construction unit shall prepare an investment estimate in strict accordance with the approved investment estimate. When examining and approving the investment budget, the Provincial Development and Reform Commission shall solicit opinions from the Provincial Department of Finance for projects exceeding the approved investment budget. If the investment estimate (or the financial capital investment) exceeds the investment estimate approved by the project by 10% or more, the provincial industry department shall reorganize the preparation of the investment estimate, and submit it for approval according to the procedures and authority specified in paragraph (2), and then give a reply.

  2. Budget adjustment. If the actual investment of major projects exceeds the investment estimate and needs to be adjusted, it shall be submitted for approval in accordance with the procedures and authority stipulated in the Notice of the General Office of the Provincial People’s Government on Printing and Distributing the Measures for the Examination and Approval of Government Investment Projects at the Same Level in Hunan Province and the Management of Budget Estimate (Xiang Zhengban Fa [2016] No.85). Among them, the need for additional provincial financial arrangements, should seek the opinions of the Provincial Department of Finance.

  (4) examination and approval of funding arrangements.

  Major projects approved by the provincial industry authorities shall apply to the Provincial Department of Finance for funding arrangements according to the construction progress and capital requirements. The Provincial Department of Finance shall review the approved investment budget, arrange funds according to the procedures and authority specified in Hunan Zhengban Fa [2015] No.90 of the General Office of the Provincial People’s Government or put forward opinions and submit them to the provincial people’s government for approval.

  The surplus funds of major projects are first used to repay the project loans. If there is a balance, according to the part of the project funds that belongs to financial funds, the finance shall be recovered in accordance with the relevant provisions of the budget management system within 3 months after the project is completed and accepted.

  Among the remaining construction funds that are terminated, scrapped or not built according to the approved construction contents, the fiscal funds confirmed according to the proportion of the actual source of funds of the project shall be recovered.

  (5) Examination and approval of debt borrowing.

  Administrative organs, institutions undertaking administrative functions and public welfare institutions shall not borrow debts. Provincial public universities, public hospitals and other public welfare second-class institutions that really need to borrow debts for the implementation of major projects shall be submitted for approval according to procedures.

  Public welfare second-class institutions shall apply for borrowing according to the approval of major projects and financing needs. The application for borrowing shall include the basis and necessity of borrowing, the analysis of the benefit of the use of debt funds, the analysis of debt repayment plan and solvency, and the measures to ensure the timely repayment of debts. After the preliminary examination by the competent department, it shall be sent to the Provincial Department of Finance. The Provincial Department of Finance, according to the unit revenue and expenditure situation, debt scale and debt risk degree and other factors, determines the total annual borrowing debt of the unit and manages it as the unit debt limit. After the new borrowing debt, the unit debt balance is controlled within the limit, and the Provincial Department of Finance makes overall consideration of the construction fund demand of major projects, determines the amount of new borrowing debt in the current year and gives a reply; If the balance of unit debt exceeds the limit, in principle, no new debt may be added. If it is really necessary to borrow money to meet the needs of its construction, the Provincial Department of Finance shall put forward preliminary opinions after the audit and submit them to the provincial people’s government for approval.

  For the debts borrowed by public welfare second-class institutions, the unit shall be the borrower and repayment subject, and bear the responsibility of loan management and debt repayment. The government is not legally liable for such debts.

  (6) Examination and approval of asset management and disposal.

  Education, culture, health institutions through the implementation of major projects to build new assets, should be in accordance with the provisions of the timely registration of property rights and fixed assets accounted for procedures.

  If the construction of major projects involves or foresees the disposal of state-owned land assets (including ground buildings and structures, the same below) in the future, the provincial industry department shall organize the project construction unit to fully demonstrate the necessity, feasibility and expected disposal income of land assets, prepare a special land assets disposal plan, and report it to the provincial people’s government for approval according to the procedures after being audited by the Provincial Department of Finance. Included in the scope of examination and approval of land assets disposal matters include:

  1. Disposal of original land assets after the overall relocation of the unit;

  2. Adjust the disposal of vacated land assets after new major projects are built;

  3, long-term idle or did not play a practical role in the disposal of land assets;

  4. Disposal of other land assets related to the construction of major projects.

  The disposal of the above-mentioned state-owned land assets (including transfer, assignment, replacement, transfer, government expropriation and storage of state-owned land, etc.) should strictly implement the disposal system of state-owned assets in administrative institutions, perform the examination and approval procedures, standardize the disposal behavior, and ensure that the disposal plan is scientific and reasonable, the asset evaluation is objective and fair, and the transaction process is open and transparent. No unit or individual may dispose of state-owned land assets without authorization if the examination and approval procedures are not fulfilled. The specific examination and approval procedures for the disposal of state-owned land assets are as follows: the disposal unit submits information such as asset disposal plan and application, and after the preliminary examination by the competent department, the audit by the Provincial Department of Finance, and the approval of the provincial people’s government, the land asset disposal reply is issued. When the disposal is implemented, the disposal unit will report the implementation plan of land assets disposal and other information to the Provincial Department of Land and Resources, which will review it jointly with the Provincial Department of Finance and submit it to the provincial people’s government for approval before issuing opinions on the implementation of land assets disposal.

  The disposal of land assets shall be carried out by means of bidding, auction, listing and agreement transfer according to laws and regulations, and the reasons shall be fully explained if the disposal is carried out by agreement transfer. Need to assess, should be entrusted with the corresponding qualification assessment agencies to assess. For the land to be disposed of without considering the nature of land use in urban planning, the local government may, on the basis of reasonable and feasible organization and demonstration, adjust the nature of land use and relevant land use control indicators, and modify the urban planning according to legal procedures.

  All income from the disposal of land assets shall be paid into the state treasury, incorporated into the overall budget arrangement, and shall not be directly returned to the disposal unit. Without the approval of the provincial people’s government, the income from asset disposal shall not be arranged as a source of funds for special construction planning, major project construction and debt repayment in advance.

  Fourth, supervision and management

  (1) The Provincial Department of Finance, the Provincial Development and Reform Commission, the provincial industry authorities and the project construction unit should strengthen the supervision of the whole process of major project construction funds before, during and after the event, and timely carry out inspections focusing on budget control to stop and correct violations of the budget. If the application is not made according to the procedure and the approval of budget adjustment is obtained, the Provincial Department of Finance shall not allocate funds if the investment of financial funds exceeds the budget.

  (two) the establishment of investment evaluation, asset management, performance evaluation, government procurement and supervision of the supporting management mechanism. The declaration, examination and approval, appropriation and use of construction funds for major projects and the disposal of state-owned land assets shall be subject to the supervision and inspection of discipline inspection and supervision, auditing, finance, land and resources, etc. If violations of rules and regulations are found during the inspection, they shall be dealt with in strict accordance with relevant laws and regulations.

  (III) If the construction unit disposes of state-owned land assets without approval, borrows debts without authorization, or misappropriates debt funds, and fails to repay debts as planned, once it is discovered, the Provincial Department of Finance shall reduce the arrangement of relevant special funds, and hold the main person in charge of the unit and the relevant personnel accountable.

  (4) Any special construction plan, major project decision-making and fund arrangement that does not involve national security or state secrets and is not prohibited by laws and regulations shall be disclosed to the public by the provincial industry department in accordance with the relevant provisions on the disclosure of government information.

  V. Supplementary Provisions

  These Measures shall come into force as of the date of promulgation. If the relevant provisions are inconsistent with these Measures, these Measures shall prevail.

Behind the successful case of "finding a needle in a haystack" among 1.4 billion people, there is a set of exciting data: since the Ministry of Public Security deployed the "reunion" operation in January, 2,609 missing and abducted children have been recovered over the years, of which the longest time span is 61 years; 147 cases of child trafficking have been detected, 372 suspects have been arrested, and more than 1,200 paternity tests have been organized in various places. Among them, after the centralized release of more than 3,000 free blood collection points nationwide on June 1, nearly 10,000 people have gone to public security organs to collect blood for free, which has helped 306 families achieve reunion.

In this way, the comparison of DNA data related to free blood collection seems to be the focus of solving the case.

But it’s not that simple. The case of the Guo family, and even Humou, a habitual child trafficker, who was dug out one old case after another in 2001, 2000 and 1997, are the epitome and "living specimen" of China’s abduction work.

A remarkable "centralized comparison battle of criminal technology"

On July 13th, the Ministry of Public Security held a press conference, which not only informed the newly uncovered case of Guo Gangtang’s son being abducted 24 years ago, but also introduced the latest progress of the "reunion" action of the Ministry of Public Security. According to Tong Bishan, deputy director of the Criminal Investigation Bureau of the Ministry of Public Security,

"On September 21, 1997, Guo Xinzhen, the two-and-a-half-year-old son of Guo Gangtang (male, 51 years old) in Liaocheng, Shandong Province, was taken away by a strange woman while playing at home, and his whereabouts were unknown. After receiving the alarm, Liaocheng Public Security Bureau set up a task force, carried out a lot of investigations, visited the platoon and other work, and collected the blood samples of Guo Gangtang and his wife, tested the DNA information and entered it into the’ abduction DNA system’, but Guo Xinzhen’s whereabouts were never found. In this year’s’ reunion’ operation, the Ministry of Public Security listed the case as a listed supervision case to organize a tough attack. "

"In June, during the centralized comparison battle organized by the Ministry of Public Security, experts were brave enough to innovate, overcome the obstacles that children’s physical characteristics were quite different from those when they grew up, broke through technical barriers, and successfully found clues suspected of Guo Xinzhen in Henan by using the latest comparison and search methods. According to the instructions of the Ministry of Public Security, Henan public security organs collected the DNA information of relevant personnel for rechecking and testing, and finally confirmed that this person in Henan was Guo Xinzhen, the son of Guo Gangtang and his wife who had been abducted for 24 years. The public security organs in Liaocheng, Shandong Province quickly sent personnel to Henan, Shanxi and other places to track and carefully investigate the adoption and trafficking situation in that year, and arrested two suspects who trafficked Guo Xinzhen in that year, and found out the facts of the case. The dust of the old case that happened in September, 1997 has settled, which is not only "unforgettable for Guo Gangtang’s family, but also inseparable from the persistence of the investigators".

Since the establishment of the Anti-abduction Office of the Criminal Investigation Bureau of the Ministry of Public Security in 2007, the Ministry of Public Security has deployed a nationwide anti-abduction campaign in 2009, established a national anti-abduction DNA system, implemented a rapid search mechanism for missing children nationwide in 2011, launched an emergency release platform for missing children information of the Ministry of Public Security in 2016, and issued and implemented phased action plans related to human trafficking many times in the State Council.

At the beginning of 2021, the Ministry of Public Security deployed the national public security organs to carry out the "reunion" action. From May 11th to June 11th, the Ministry of Public Security organized and launched the "Centralized Comparison Battle of Criminal Technology in Reunion Action" in Jinan, Shandong Province. Through a month’s comparison of clues found in the battle, 718 missing and abducted children have been recovered and 8 escaped children have been arrested, which has achieved remarkable results.

The comparison battle mainly has the following characteristics:

First, the combatants are fine. The Ministry of Public Security has dispatched 69 criminal technical experts from public security organs all over the country. These experts are comprehensive talents, who know their major and know how to judge, and have rich practical experience.

Second, the data resources are complete. The action extensively collected photos, DNA and other information of missing and abducted children and their parents, and studied and formulated the technical comparison work specifications to ensure that the relevant search and comparison data were complete.

Third, the system has strong functions. The "abduction DNA system" was upgraded in time, and at the same time, the "reunion action technology comparison battle platform" was specially developed to realize the special work such as verification of parents’ identity information, verification of suspected abductees, upload and release of work orders, and review statistics of action results, which provided functions such as case management, multi-mode comparison, comprehensive analysis, expert consultation and task push for comparison battles. During the battle, the experts involved in the war expanded the application of the latest scientific and technological means, and timely connected with the field visits and investigations, ensuring the success of the battle.

In the next step, the Ministry of Public Security will conscientiously sum up the successful experience and practices of the first comparison battle and organize the second centralized comparison battle as soon as possible.

How efficient is the "abduction DNA system"?

In the "daily abduction", the "DNA system of abduction" established in 2009 has attracted the attention of many netizens. This time, the police successfully found a clue that suspected Guo Xinzhen in Henan by using the latest comparison and search means, and then collected his DNA information and entered it into the "DNA system of abduction", successfully compared it with Guo Gangtang and his wife in Liaocheng, Shandong Province, and finally confirmed Guo Xinzhen’s identity.

On June 1, the Ministry of Public Security announced more than 3,000 free blood collection points across the country through news media and other channels. Blood collection is not restricted by household registration. Parents of missing abducted children who have not taken blood, suspected abducted persons and persons with unknown origin are urged to collect blood at nearby blood collection points for free as soon as possible and rely on science and technology to find relatives.

How effective is this?

In recent years, there are often cases in which the public security organs blindly compare the missing abducted children in the DNA information database. The abducted people didn’t even know that their life experience was suspicious. After June 1, nearly 10,000 people took the initiative to receive free blood collection from public security organs, and now 306 families have been reunited.

On the 13th, the police chief V@ Jiangning’s mother-in-law posted that the parents of abducted children must record their parents’ DNA for free after calling the police, and the children found by the police after abduction and rescue must record their children’s DNA for free. For the police situation of trafficking before 2009, over the years, local public security organs have also continuously recorded DNA, and basically there are few omissions. "As long as the parents have reported the police to collect DNA, it is basically only a matter of time before finding the child."

In addition, "under the combination of public security organs’ investigations and heavy punches, the cases of child trafficking have shown a significant decline or even almost disappeared in recent years. Most of the missing children who are recovered every year are lost or run away from home. At present, they mainly focus on finding accumulated cases. "

In 1963, the 5-year-old boy "Ji Xian" fled to Henan with his family and then separated. After a lapse of 58 years, the "reunion" action of the Ministry of Public Security made his 90-year-old mother finally wait for the news of her son in her lifetime.

In addition, @ Jiangning’s mother-in-law also mentioned the difficulty in the current abduction work, saying that quite a few of the children rescued by local public security organs were not comparable to their parents’ DNA in the database. The only possibility is that the parents of the children have not reported to the police, and most of them have taken the initiative to sell their own children.

Going back to the 24-year-old case, the public security organs followed the line around Guo Xinzhen’s adoption in that year and identified two suspects, Hu and his girlfriend Tang. At present, the case is still being further expanded.

In fact, Hu Mou, a 56-year-old recidivist of child trafficking, can also be regarded as a "living specimen" of the effectiveness of China’s abduction work.

According to media reports, in 1988 and 1996, Hu was jailed for theft and extortion respectively.

In 2009, Hu was arrested for allegedly abducting a girl in Hebei in 2001.

In 2021, Hu was arrested and brought to justice by Shanxi police for allegedly abducting a boy in Shanxi in 2000. Because after the "Reunion" campaign was launched this year, the blood samples collected by the police from the parents of abducted boys were more successful than a suspected abducted child in Zhonghenan. After careful investigation, the police locked two suspects, including Hu.

By June of this year, important progress had been made in the abduction of the son of Guo Gangtang, the prototype of the movie "Lonely Lost", and the suspect Hu was seriously afraid of sin when he was arraigned, and refused to confess. The task force conducted further investigations around its related parties. After his girlfriend Tang confessed the crime, Hu finally pleaded guilty.

Therefore, Hu added another case in 1997. This further shows that the recidivist Hu has never voluntarily confessed the crimes he has committed.

Many generic pharmaceutical companies will inevitably withdraw from the historical stage in the wave of supply-side reform in the era of high-quality development. The future pharmaceutical industry ecology may be like this:

First, 3-5 head pharmaceutical companies with sales or market value exceeding 100 billion.This should include generic drug predators, giant central enterprises, innovative drugs upstarts and senior traditional and established pharmaceutical companies. TOP1 pharmaceutical companies may exceed 200 billion sales or market value.

The second is about 30 waist pharmaceutical companies with sales or market value of 10-50 billion yuan.There are mainly traditional established pharmaceutical companies (generic drugs+innovative drugs), innovative pharmaceutical companies and central enterprises, and there are almost no pure imitation pharmaceutical companies.

The rest are about 300-500 small and beautiful companies.Sales or market value is not large, mainly concentrated in a certain field such as rare diseases, anti-infection, tumors, mental nerves and so on. A company that has developed a single product but caters to the needs of the development of Chinese disease spectrum may be a small and beautiful pharmaceutical company. Many of these companies have developed from Biotech, and there will be no traces of imitation pharmaceutical companies at all.

The research and development of innovative drugs itself is characterized by long cycle, high risk and large investment. Some private enterprises are not strong in financial strength, and state-owned enterprises have always been less enthusiastic about investing in innovative drugs because of mechanism reasons.Therefore, it is probably the fate of many generic pharmaceutical companies to continue to cultivate generic drugs.

If so, the future competition seems very realistic and cruel. But this is the future after all. How can the current generic pharmaceutical companies tide over the difficulties and achieve transformation? How to live with time for space in the transition period?

The way out for generic pharmaceutical companies

The first is to protect the existing approvals.The domestic pharmaceutical market is gradually returning to the era of "product is king" from "marketing is king" and "market is king". Drug approval is the most important asset of pharmaceutical companies. For generic drugs, consistency evaluation must be done as much as possible, and the evaluation should be exhausted. copyThe consistency evaluation of drugs is changing from the previous product life and death symbol to the amulet of future generic drugs.

The product assets of future generic pharmaceutical companies are mainly reflected in the number of products that have passed the consistency evaluation. Even if you now have 500 generic drug approval numbers, but only ten have passed the conformity evaluation, then the future assets are these ten, plus the products that cannot be evaluated in conformity. Because the policy is getting tighter and tighter, medical institutions will stop selling products that have not been evaluated after the implementation of national procurement.Please note that medical institutions stopped selling, not the internal standard market.. 

Many of the generic drugs mentioned just now that cannot be evaluated for consistency are because there is no reference preparation. Why is there no reference preparation? This may be related to the China characteristics of new drug development in previous years. This kind of products are similar to many exclusive products of Chinese patent medicines. At present, there is no clear statement on how to manage them in the future, but the author is not optimistic about this kind of products (many of them are so-called exclusive products, exclusive dosage forms and exclusive specifications).

The second is to seize the time window to do the research and development of imitation new drugs, and exchange time for space.

Consistency evaluation+centralized procurement has reshaped the domestic drug business logic, which has given birth to many opportunities, including products that have been adopted in China. Some enterprises have achieved overtaking in corners because of winning the bid; Some pharmaceutical companies cater to the policy trend and quickly develop suitable products to fight for national adoption, and they have also done a good job.

Crisis crisis has always been the coexistence of "danger" and "opportunity". Imitation pharmaceutical companies should make research and development projects of generic drugs with optimistic attitude, keen insight, professional judgment and moderately advanced investment awareness.

In this round of centralized procurement, pharmaceutical companies in Shandong, Sichuan, Hebei and Zhejiang have actively participated in the national procurement because of their unfocused consistency evaluation, and have basically established the market position of future head imitation pharmaceutical companies. This fact should arouse the profound reflection of other generic pharmaceutical companies, and reflect on their own sensitivity in understanding national policies and their gains and losses in the company’s development strategy.

The reality is that due to policy changes, the valuation of generic drugs has dropped by 1/2 to 4/5, and the development of generic drugs is becoming more and more difficult. It is a job of catching mice in porcelain shops and embroidering needles. But there is still a chance,It is to seize the development and fight for the country.

Since 2022, the author has cooperated with the company’s R&D department to intensively investigate more than 100 projects for the establishment of generic new drugs. Most of them are worthless, but some projects still have good business prospects. Even in the face of various policy changes in the future, the deduction results of our sales scenarios are optimistic, and we feel that the risks are relatively controllable. Specifically, the current research and development of generic drugs needs to pay special attention to the following dimensions.

Five dimensions of generic drug research and development project

First, the stock market should be large enough.The biggest charm of national mining lies in the quantity and the reshuffle of the stock market. Therefore, the stock market is an important dimension. If the stock exceeds 1 billion, about 500 million will be valuable, and the market of 100 million will depend on listed manufacturers, consistency evaluation and the number of R&D companies. Those below 100 million should pay attention to risks.

Second, there are as few listed manufacturers as possible.This is the same as the previous generic drug research and development project, "first imitation as much as possible."

Third, there are as few manufacturers under research as possible.This is the same as before.

Fourth, the bid price is competitive without national mining or centralized mining.Because the purpose of development is to fight for national adoption, it is easy to understand that there is no national adoption. However, the reality is that it has been almost four years since the national drug collection was carried out. Under the background of "all drugs should be collected", I am afraid that most commonly used clinical drugs will be collected in the future. It doesn’t matter, there is also gold in the products that have been collected. Because of the introduction of the national mining renewal rules, sometimes it has unique value, which requires our discerning eyes to gain insight and consider various scenarios at the same time when establishing the project.

The fifth is to investigate the market access attributes of products such as medical insurance and basic drugs.Judging from the previous batches of national procurement, the probability of national procurement of non-medical insurance products is small, and the basic drugs are all medical insurance products, while the policy orientation is that the attributes such as medical insurance basic drugs are becoming more and more important in the standard market. Are these lines particularly wrapped around? No way, the project of generic drugs is to fully consider the various possible combinations of each generic drug label and deduce various possible scenarios in the future.

The supply-side reform of medicine cannot be achieved overnight, and it will take a long time to establish the era of high-quality development of medicine. The reshaping of the pharmaceutical industry ecosystem mentioned above may come when the strategy of "Healthy China 2030" is reached.

Therefore, it is the only way for generic pharmaceutical companies to break through the current difficult period by seizing the time to do a good job in the consistency evaluation of generic drugs, research and development projects, and trading time for space.
